Wolverhampton Voluntary Sector Council and the Clinical Commissioning Group are working together on an exciting pilot project that will lead to more people benefitting from the support of the Voluntary and Community Sector in our City.
GPs and other primary care providers will be supported to refer patients to a range of support that is available across the City in order to help those patients look at alternative ways of achieving improved health and well- being.
A number of Link Workers will be based with partners across the City – their job will be to develop stronger links between Primary Care providers, their patients and the City’s Voluntary and Community Sector .
This project is part of the transformation of health and social care provision that is so necessary to halting the increase in demand for our excellent Health Service , for example by enabling a more pro-active and preventative approach to health and well being
Wolverhampton Voluntary Sector Council already has a comprehensive list of organisations working in the Health and Social Care field and we will be contacting you soon. We will also be contacting GPs and other Primary Care providers across the City to talk about this innovative work.
